To change your name, please visit any financial center with the following documentation:

  1. Social Security card with your updated name
  2. Driver's license with your updated name
  3. Marriage certificate (if applicable)

A request to change your name can also be faxed or mailed to Allegacy.  We will need a written request with your member number, old name, new name, and your signature.  Please fax/mail the written request with a copy of the above documentation.  The request can be faxed to Member Services at (336) 774-4115 or (800) 844-6464.  The request can also be mailed to PO Box 26043, Winston Salem, NC 27114
